Transcripts for Alumnae

This transcript request form is used for all alumnae who are looking to send a copy of their official transcript to a college or potential employer. The form must be completed and signed by the former student. The turn around time is usually 1-2 days upon receiving the transcript request. There is no fee for this service.

Transcripts for Current Students

The electronic transcript request form is used when your college/university will accept transmissions electronically. The Counseling Department sends all documents (school profile, transcripts, and letters of recommendation) through Naviance. This form requires both a student and parent signature. We require three weeks (15 school days) to process transcript requests. It is the responsibility of the student to be aware of the application deadlines for each college included on the form.

The manual transcript request form is used when a college/university does not accept electronic submissions and requires that materials be mailed to the college. Work Permit

Many students at Our Lady of Mercy School for Young Women balance their academics, athletics, and club participation with a part-time job, particularly in the summer. The decision to work is one that needs to be made by the student and her parents/guardians as there are many things to consider.

Mercy’s guidance counselors are more than happy to speak with a student and her parents regarding the benefits and concerns of paid employment during the school year or during summer break. Mercy students who are under 18 must receive a work permit prior to beginning most jobs. Please follow the steps below to acquire a permit.


Work Permit Guidelines

  1. Work permit form AT-17 (link below) needs to be completed and signed (only parts 1 and 2 need to be completed). The form must have a parent signature.
  2. Check with the nurse to verify that the school has a current health form on file. (During the summer, this information will be available in the Counseling Office).
  3. Bring the completed form to Ms. Costello in the Counseling Office. The work permit card will be given to the student. NOTE! The student must be present to sign the work permit card.

*Parents are not permitted to pick up the card for their daughter, as it must be signed by the student herself.
